• Physical Exams

    Pennsylvania requires a complete physical examination upon entrance into school (kindergarten or first grade), and in grades six and eleven. Students new to Pennsylvania schools must have a physical exam if the student's record does not meet Pennsylvania requirements.
     
    Parents are urged to take their children to their own physician for examinations to provide continuity of care. Your family physician is familiar with your child and is better able to detect any physical change, to follow through with needed care, and immunization boosters.
    If desired, the school doctor can perform the needed physical examination required by PA state law. Usually the exams are performed in late spring.
     
    Please note:  This exam form cannot be used for PIAA sport's participation Grades 7-12.  Paperwork for PIAA participation can be found  on your child's school home page under Athletics.
     
     
     

     
    Dental Exam / Program

    The Public School Code of Pennsylvania requires all children in grades Kg, 3 & 7 to have a dental examination. Students new to Pennsylvania schools must have a dental exam if the student's record does not meet Pennsylvania requirements. Examinations completed by a family dentist within four months of the start date of the school year are accepted for the required proof.
